This
Directory was developed with support from a cooperative agreement (U93
MC00133) Introduction A
primary mission of the Institute for Child Health Policy is to facilitate
coordination and collaboration among federal, state, and local agencies
and organizations that address the needs of children and families.
In keeping with this mission, and with support from a grant from
the Maternal and Child Health Bureau, the Institute has developed this
Directory of Eligibility Criteria and Scope of Services of Title V CSHCN
Programs in order to support coordinated outreach and referral activities
and to assist state CSHCN Programs to compare their activities with their
counterparts throughout the nation.
State
Title V CSHCN Program staff has reviewed the draft profiles of their
program to assure accuracy. However,
there is detailed and important programmatic information that could not be
included in this directory and changes may occur at any time.
Therefore, specific questions about eligibility and services should
always be addressed directly to the State CSHCN Programs. A
hard copy of this Directory is being made available to federal and state
organizations serving children with special health needs and their
families. In addition, the
information contained in this Directory is available through the Institute
for Child Health Policy World Wide Web (WWW) page, which is supported by
MCHB. This
Directory, as well as state-specific profiles, is maintained on the WWW.
Changes to state profiles will be made available on the WWW as soon
as that information is received. The
